Eligibility Criteria
To qualify for our services, individuals must meet the following:
Primary diagnoses of serious mental illness
Age 18 years or older
Income below 150 percent of the federal poverty level
Able to live in a community with program services
Receiving Medicaid or eligible post-state hospital discharge.
